Overview
ATN Healthcare is committed to offering a benefits package that supports our workforce while ensuring the agency’s operational efficiency and financial stability. The following benefits plan outlines eligibility, enrollment, and specific offerings for eligible employees, effective as of July 1, 2025.
Eligibility Criteria
To qualify for the ATN Healthcare Benefits Plan, employees must meet the following criteria:
- Employment Status: Full-time employees (working 30+ hours per week) or part-time employees (working 20–29 hours per week) under specific roles designated by management (e.g., registered nurses, administrative supervisors).
- Tenure: Employees must complete a 90-day probationary period before becoming eligible for most benefits, except where noted.
- Performance: Employees must maintain a satisfactory performance rating (as determined by annual reviews) to remain eligible for discretionary benefits (e.g., profit sharing, company car).
- Compliance: Employees must adhere to agency policies, including mandatory training and client satisfaction standards, to retain benefits eligibility.
Enrollment Period
- Open Enrollment: Annual open enrollment occurs from November 1 to November 30, with changes effective January 1 of the following year.
- New Hire Enrollment: Eligible new hires may enroll within 30 days of completing the 90-day probationary period.
- Qualifying Life Events: Employees may enroll or modify benefits outside open enrollment due to qualifying events (e.g., marriage, birth) within 30 days of the event, subject to management approval.
Kick-Off Dates
- Plan Effective Date: July 1, 2025, for all benefits unless otherwise specified.
- Specific Benefit Rollouts:
- Health, Dental, Vision, and Life Insurance: July 1, 2025
- 401(k) and Employee Private Stock Purchase Plan: August 1, 2025
- Profit Sharing and Adoption Assistance: January 1, 2026 (for eligible employees)
- Other benefits (e.g., mileage reimbursement, referral program) are available upon eligibility post-probationary period.
Benefits Details
- Health Insurance
- Description: Comprehensive medical coverage through a preferred provider organization (PPO) plan, with ATN Healthcare selecting the carrier to ensure cost efficiency.
- Details: Covers 70% of premiums for employees; dependents may be added at the employee’s full cost. High-deductible plan options are available to reduce agency costs.
- Conditions: Employees must work a minimum of 30 hours per week and complete annual wellness screenings to maintain coverage. Non-compliance may result in coverage suspension.
- Paid Time Off (PTO)
- Description: Accrued PTO for full-time employees, capped at 120 hours annually (15 days).
- Details: Accrual begins after 90 days at a rate of 4.62 hours per biweekly pay period. Unused PTO is not paid out upon termination.
- Conditions: PTO requests must be approved by management based on staffing needs.
- 401(k)
- Description: Retirement savings plan with a 2% employer match after one year of service.
- Details: Employees may contribute up to IRS limits. Matching is contingent on agency profitability and capped.
- Conditions: Vesting occurs over 5 years (20% per year).
- Referral Program
- Description: Bonuses for referring qualified healthcare professionals who join ATN Healthcare.
- Details: $500 bonus per successful hire, paid after the referred employee completes 6 months of service.
- Conditions: Referrals must meet performance standards. Bonuses are taxable and subject to agency discretion.
- Continuing Education Credits
- Description: Reimbursement for approved continuing education courses relevant to job roles.
- Details: Up to $1,000 annually, subject to pre-approval and budget availability.
- Conditions: Courses must align with agency needs. Employees must remain employed for 12 months post-reimbursement or repay the full amount.
- Mileage Reimbursement
- Description: Reimbursement for work-related travel at the IRS standard rate (2025 rate: $0.67 per mile).
- Details: Applies only to pre-approved travel for client assignments. Monthly cap of 500 miles per employee.
- Conditions: Employees must submit detailed mileage logs within 30 days of travel.
- Dental Insurance
- Description: Dental coverage for preventive and basic services.
- Details: Agency covers 60% of premiums; employees cover orthodontics or major procedures. Annual benefit cap of $1,500.
- Conditions: Enrollment tied to health insurance plan.
- Vision Insurance
- Description: Coverage for eye exams, glasses, and contacts.
- Details: Agency covers 50% of premiums with an annual benefit cap of $300.
- Conditions: Must be enrolled in health insurance plan.
- Life Insurance
- Description: Basic life insurance policy (1x annual salary, up to $50,000).
- Details: Fully funded by ATN Healthcare for full-time employees after 90 days.
- Conditions: Employees must designate beneficiaries and update annually.
- Employee Discount
- Description: Discounts on healthcare-related services through agency partners.
- Details: Up to 15% off select services, subject to partner agreements.
- Conditions: Discounts are non-transferable and limited to active employees.
- Opportunities for Advancement
- Description: Internal job postings and leadership training programs.
- Details: Priority given to employees with 2+ years of service and exemplary performance.
- Conditions: Advancement is contingent on agency needs and budget approval.
- Flexible Schedule
- Description: Option for flexible hours for select roles.
- Details: Schedules must align with client needs and receive supervisor approval.
- Conditions: Not available for direct-care roles.
- Relocation Assistance
- Description: Reimbursement for relocation expenses for critical roles.
- Details: Up to $2,000 for approved relocations, paid after 6 months of service.
- Conditions: Employees must sign a 2-year commitment or repay funds.
- Employee Private Stock Purchase Plan / Private Stock Options
- Description: Opportunity for select employees to purchase company stock or receive stock options.
- Details: Available to employees with 3+ years of service and executive approval. Stock options vest over 4 years.
- Conditions: Limited to 1% of total shares annually. Buyback rights reserved by ATN Healthcare.
- Adoption Assistance (If Eligible)
- Description: Financial assistance for adoption expenses.
- Details: Up to $5,000 per adoption, available to full-time employees after 2 years.
- Conditions: Subject to annual budget and executive approval. Employees must remain employed for 2 years post-assistance or repay funds.
- Paid Training / Paid Orientation
- Description: Paid training and orientation for new hires and ongoing skill development.
- Details: Up to 40 hours of paid training annually, focused on agency-specific protocols.
- Conditions: Training is mandatory and scheduled at agency discretion.
- On-Site Gym
- Description: Access to an on-site gym at select ATN Healthcare facilities.
- Details: Free for full-time employees; part-time employees pay a $10/month fee.
- Conditions: Usage is subject to facility hours and availability.
- Profit Sharing (If Eligible)
- Description: Annual profit-sharing bonus for top performers.
- Details: Up to 2% of net profits distributed among eligible employees, based on performance and tenure (3+ years).
- Conditions: Subject to board approval and agency profitability.
- Cell Phone
- Description: Company-provided cell phone for select roles requiring client communication.
- Details: Basic models provided; employees cover personal usage costs.
- Conditions: Phones remain agency property and must be returned upon termination.
- Company Car (If Eligible)
- Description: Access to a company car for senior field staff in high-travel roles.
- Details: Available to employees with 5+ years of service and specific job requirements.
- Conditions: Usage is tracked, and personal use is prohibited. Vehicles are leased.
- Malpractice Insurance
- Description: Coverage for professional liability for clinical staff.
- Details: Fully funded for full-time clinical employees after 90 days.
- Conditions: Coverage is limited to agency-approved activities.
- On-the-Job Training
- Description: Ongoing training during work hours to maintain certifications and skills.
- Details: Provided at no cost to employees, scheduled based on operational needs.
- Conditions: Mandatory participation; failure to complete may impact benefits eligibility.
- Other Benefits
- Description: Additional perks such as employee recognition programs, team-building events, and wellness initiatives.
- Details: Budgeted annually at agency discretion (e.g., $500 per employee for events).
- Conditions: Participation is optional but encouraged.
Administration and Disclaimers
- Plan Administration: ATN Healthcare reserves the right to modify, suspend, or terminate any benefit at its discretion, with 30 days’ notice to employees.
- Cost Control: Benefits are subject to annual budget reviews to ensure financial sustainability.
- Employee Contributions: Where applicable, employees contribute to premiums or costs to offset agency expenses.
- Termination of Benefits: Benefits cease immediately upon termination, except for vested 401(k) contributions or legally mandated coverage (e.g., COBRA).
