Cost-effective care services
ATN Healthcare provides various health and human services through our Home & Community-Based Services. We offer person-centered care to provide individuals with functional or cognitive limitations.
Our HCBS waiver programs are targeted at seniors and people with disabilities. We also provide opportunities for Medicaid recipients to acquire long-term services and support in their own home.
The types of Home and Community-Based Services provided include:
- activities of daily living (ADLs) like toileting, bathing, eating and getting in and out of bed
- case management services (e.g., medical service coordination),
- home meal deliveries
- transportation
- financial and legal services
- employment and work services
- engagement in community life
- personal resources
